If it is all tarnish (and not shaded designing) then you need a liquid tarnish remover that you can dip the necklace into - assuming it has a lot of convoluted surfaces that make polishing it with cream a real hassle. Or you can take it to a jeweller with proper cleaning machines and solutions.

If you cleaned it with a cleanser to remove the tarnish that could be making you break out.
Or you could have developed an allergy to silver. This happens sometimes.
Try wrapping the necklace around your wrist or hand and see if you break out there - it could be something else that's making your neck break out but the necklace was irritating it. So try that experiment.
Don't put anymore alcohol on it. Just wash it with some dishwashing soap and then let it completely air dry.
If it is something you put on it this should remove it.
If it is the actual silver you can be happy that they make beautiful chains in surgical steel that never tarnish. hold up beautifully and are very pretty to wear.
